RICHMOND, KY. (July 29, 2026) — The Richmond Police Department is saddened to confirm the passing of eight-year-old Addy Maggard of Lesage, West Virginia, following a tragic collision near Interstate 75’s Mile Marker 77 on Tuesday afternoon. The incident involved multiple individuals and prompted an emergency response from the Berea Police Department, Richmond Police Department’s Collision Reconstruction Unit, Madison County Coroner’s Office, Deputy Coroner Carlos Coyle, and additional emergency response agencies who worked together at the scene.
The collision occurred on July 28, 2026, at approximately 2:00 p.m., when the Berea Police Department responded to reports of a serious crash involving two vehicles that had collided and were blocking traffic along Interstate 75. Upon arrival, officers immediately began providing first aid and assistance to the occupants involved in the crash while working to secure the scene and manage the impact on interstate traffic.
Due to the circumstances surrounding the collision, the Berea Police Department requested assistance from the Richmond Police Department’s Collision Reconstruction Unit. Investigators responded to assist with documenting the scene, gathering evidence, and beginning the process of determining the events that led to the crash.
Eight-year-old Addy Maggard of Lesage, West Virginia, was transported by emergency responders to St. Joseph Hospital in Berea, where she was later pronounced deceased by Deputy Coroner Carlos Coyle of the Madison County Coroner’s Office. Despite the efforts of first responders and medical personnel, Addy tragically succumbed to injuries sustained in the collision.
Two additional occupants involved in the crash were also transported to local medical facilities for treatment. Their conditions are not being released at this time, and officials have stated that further details will only be shared when appropriate and permitted.
The investigation into the cause of the collision remains ongoing. The Richmond Police Department, Berea Police Department, Madison County Coroner’s Office, and all responding agencies continue working to review the circumstances surrounding the incident and determine any contributing factors.
